I think you need to read the book, as that explains the brief non sequitur shot which shows the vampire child plainly is a girl.

Just watched The Boy In The Stripped PyjamasOne of the most harrowing films I have seen in quite awhile because it is so subtle in how things are portrayed - such terrible horrors being seen through the eyes of a child. It could be said that the camp commandant gets his just deserts in what happens but Bruno is such a lovely innocent, caring child............ I take my hat off to the writer of this book for such a kick to the guts ending.
