Only the first bar on the left was fully lit up, when I first went in. The other two, on the right, were blank.

You need to set PCM to 86% (0dBFS - no digital gain or attenuation)
Playback and Playback boost are the ones that should be adjusted to suit (according to the input sensitivity of your pre) this will be an ears and brains activity. I'd suggest setting the pre to a sensible level and then winding up the 'playback' control, if you get all the way to the top and your system is still quiet (compared to what you'd expect for the pre volume setting) then proceed to winding up the playback boost control too (there's another couple of milivolts of gain on this slider) you'll want to be listening for any distortion that might be coming from overloading the inputs to your pre, if there is none then you're on to a winner.

The fact that those bars were blank may help answer why you were having the low volume issue earlier.
