The proposed cover was of the space shuttle exploding but in 1987 that was deemed 'too soon' so it was replaced with the cover shown above.
Just like with Spinal Tap's 'Smell The Glove' a number of major retailers refused to stock the album and the cover was redesigned again although not until many of the original had made it into the wild. Having discovered the band about a year before the album was released I got it as soon as it came out so have the 'offensive' cover on my vinyl copy although these days just play the CD that has the below cover:

In someways the new cover is, to me, just as bad. I dislike images of death's head, skeletons as much as I do of Christian iconography. I put up with 'momento mori' imagery as I like the work of The Greatful Dead, but since I don't like heavy metal, or death metal groups, I can largely avoid the motifs and symbolism.
