Yes the Hays Code will be the reason for the bloodless gunfights, but doesn't explain the fistfights.
Like for example the scene in 'Treasure Of The Sierra Madre' where Bogart and the other bloke fight the gang master to get their wages. It looks like they are fighting for real.
I suspect it is because the actors, director and the stuntmen and stunt arrangers all knew what a real fight looked like as they were from an age where settling disputes with violence was common. But it's just my theory.
