The issue was the iOS 'Smiley' in the subject. Interpreted by the firewall as posssibly something more troublesome than it actually is... lookup SQL Injection.

Looking at the logs I think this predated the change I made to resolve the issue (you will have been sent an email at 21:01 last night.
I can't see any 'suspicious' failed login attempts since 22:15...
