I can figure what happened from the top picture. He was too close to the building to extend the outriggers far enough. What a dick! :)

I doubt there'll be much change out of a million or two after the recovery costs and building repair work!

You'd think there'd have been an engineer on site who'd have spotted the danger and stopped the work. I would have done.
