Quad 405 4 pin RCA lead

Has anyone got a simple diagram that I could use to make one of these leads up?

The 4-way DIN socket on the 405 has the pins marked as follows (going clockwise):

1 Left-hand channel signal

2 0V signal ground

3 Right-hand channel signal

4 (not used)


The 4-way DIN plug should be wired in the same fashion.
