The BTE Designs (http://www.ebay.co.uk/itm/bte-designs-passive-pre-preamplifier-preamp-valve-or-solid-state-amps-/251447638499?pt=UK_AudioTVElectronics_HomeAudioHiF i_Amplifiers&hash=item3a8b7279e3) passive fits your criteria, just spec it with an extra set of inputs and a Dale stepped attenuator for best results. Even though the resulting unit will exceed your budget slightly I can vouch from experience that these upgrades take the unit to a different level of performance which even surpasses that offered by the mighty Tisbury Audio passive-pre... ;)
