Bought new from analogue seduction 12 months ago. It sounds very, very good and is dead easy to fit (Just remove the cover and connect 1 plug inside)

Of course, apologies for omitting them. (and it's an MM stage, btw!) I bought it brand new 1 year ago and it's been in my amp since, so really it's just identical to this (the small plug-in board on the bottom left) :
